PURVIS CHIROPRACTIC believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. WE accept most insurances as well as we accept all major credit cards, care credit, HSA cards, cash and checks.
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare and Medicaid. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge.
Medicare doesn't cover for Exam fees which are done on EVERY first visit.
"Medicaid Chiropractic services payable by Montana Medicaid are limited to those provided to children age 20 and under (also referred to as EPSDT) for medically necessary services. Medicaid does not reimburse for any chiropractic services provided to individuals age 21 and over."
Cash patients, Co-payments, deductibles, and/or coinsurance payments are due at the time of service. Insurance companies require that we collect these fees because they are the terms of your health care contract. Ultimately, the patient is responsible for all balances. Additional payments after insurance has been received must be made within 90-days after that a 5% service charge will be added. After 120-days it will be sent to collection if no payment is made.
For your convenience, we do accept credit cards including Visa, Discover, MasterCard, American Express, Debit, HSA cards and Care Credit. Payments may be made over the phone.
Due to the constant changes in the health care system, it is your responsibility to know your health care coverage. While we can look up your benefits, it is beneficial to call your own insurance when you have any questions regarding your coverage.

Medical Forms and Records
The doctors would be delighted to fill out any medical forms for patients, however there must be ample time given. Please give approximately one or two business days for the doctors to fill the forms and make sure they are completed correctly. There may be a charge of up to $25.00 per form. If you wish to release your records, you must fill out a release form at the office you wish your records to be sent to. It takes approximately seven to ten business days for requests to be handled.
